Telehealth Services

Balanced Health NP provides functional medicine services via telehealth to patients in Pennsylvania and Florida. By using our telehealth services, you agree to the following:

• You understand that telehealth involves the use of electronic communications to enable healthcare providers to deliver services at a distance
• You understand that telehealth has potential benefits (convenience, access to specialists) and limitations (inability to perform physical examinations)
• You are responsible for ensuring you have adequate internet connectivity and a private environment for telehealth visits
• You understand that technical difficulties may occasionally disrupt sessions
• You agree to be physically located in Pennsylvania or Florida at the time of your telehealth appointment
• You consent to the electronic transmission of your health information for the purpose of providing care

Appointments & Scheduling

Appointments are scheduled through our online scheduling system (OptiMantra). By scheduling an appointment, you agree to:

• Provide accurate and complete personal and health information
• Arrive on time for scheduled appointments (for telehealth, be logged in and ready at the scheduled time)
• Notify us as soon as possible if you need to reschedule

Cancellation & No-Show Policy

• 24-hour cancellation policy: We require at least 24 hours’ notice to cancel or reschedule an appointment
• Late cancellations (less than 24 hours’ notice) may be subject to a cancellation fee
• No-shows — failure to attend a scheduled appointment without notice may result in a fee equal to the full appointment cost
• Repeated no-shows may result in the requirement of prepayment for future appointments

Payment Terms

• Balanced Health NP is a cash-pay (self-pay) practice and does not bill insurance directly
• Payment is due at the time of service unless otherwise arranged
• We accept major credit cards, debit cards, HSA, and FSA payments
• Superbills are provided upon request for patients seeking out-of-network insurance reimbursement; reimbursement is not guaranteed and is determined by your insurance provider
• Laboratory testing, supplements, and other recommended services may incur additional costs beyond appointment fees
• Package pricing is non-refundable once the initial appointment has been completed; unused appointments within a package may be forfeited if not used within 12 months of purchase

Scope of Practice

Melissa Tomon, CRNP provides functional medicine services within her scope of practice as a Certified Registered Nurse Practitioner. Our services complement — but do not replace — your relationship with your primary care provider or specialists. We may recommend coordination with other healthcare providers as appropriate.

Limitation of Liability

To the fullest extent permitted by law:

• Balanced Health NP, Melissa Tomon, CRNP, and associated staff shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our website or services
• We do not guarantee specific health outcomes from our services
• Our total liability for any claim arising from our services shall not exceed the amount you paid for the specific service giving rise to the claim
• We are not liable for damages arising from technical failures of telehealth technology, internet service interruptions, or unauthorized access to electronic communications
